Before treatment
This Information Formย (pdf) which references my GDPR Privacy Policy needs to be completed before we can begin treatment. If you have a printer, then fill in these forms either digitally or on paper once printed and bring it to your appointment (do not email it, as email is not a secure medium for sending personal information). If you do not have a printer, I will give you a paper copy when we meet.

Note. Please avoid coffee and other stimulants before treatment as it makes it difficult for your body to settle into a therapeutic state.
